Job Description:
Provide , and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
Qualifications
Current, unencumbered state license to practice as a Medical Social Worker (MSW)
Minimum one (1) year of home health MSW experience required
Demonstrate effective oral and written communication skills, Computer Literate
Current CPR certification
Copy of updated Physical, based on state requirements
Copy of updated TB results/ Chest x-ray
Reliable transportation
