Qualifications Needed:
Current OK State RN Licensure, 1-2 years Home Health Experience Preferred
OASIS C-2 Training
Good Organizational, Time Management and
Communication Skills
Responsibilities:
Completes Initial Assessments (SOC) to determine home care needs.
Develops a Plan of Care which establishes goals based on nursing diagnosis and incorporates therapeutic, preventative, and rehabilitative nursing procedures.
Regularly evaluate plans of care and Performs Recertifications every 60 days.
Educate patients and their families on Plan of Care, Disease Management, Medication Management; all other aspects of Care.
Coordinates Patient Care with Physicians offices, other referral sources, community resources.
Participates in On-Call Duties as defined by On-Call Policy.
