As our growth continues, Utah Home Health and Hospice is seeking a Hospice Community Liaison to aid in our mission: to provide cost-efficient, quality hospice care to the patients entrusted in our care. We are here to exceed the expectations of our patients, clients, and their families by providing excellent clinical care and premier service.
As a Hospice Community Liaison you will:
Develop and maintain community relationships to promote hospice services.
Serve as a resource and provide information and education to referral sources, potential referral sources and the community.
Meet with referring physicians and referral sources to ensure customer satisfaction.
Educate referral sources about available hospice services.
