Service Coordinator Job Opening in Ambridge, Pennsylvania - The Service Coordinator?s primary function is to provide effective case management for consumer?s served on the Pennsylvania Aging Waiver. Service Coordinator?s help assure that all consumers receive high quality care and services in accordance with professional standards, state and federal rules and regulations and operational policies. RESPONSIBLE TO Administrator, Service Coordinator Supervisor and Governing Body. EDUCATION, QUALIFICATIONS, AND CREDENTIALS Must possess a valid Pennsylvania Operator?s License and current, up-to-date insurance coverage on vehicle. Must be detail oriented, possess good verbal and written communication skills and have a basic knowledge of Pennsylvania Waiver Services. Effective time management and organizational skills are necessary. Must possess a basic understanding of Microsoft Office and efficient typing skills. ESSENTIAL JOB FUNCTIONS The employee must be able to perform each essential function effectively to be successful in this position .
Maintain a high degree of professionalism and adhere to regulations regarding consumer confidentiality at all times.
Assist with maintaining an accurate and current filing system for each consumer.
Handle an expanding caseload.
Manage services via Waiver agencies and ensure that consumer receives appropriate services accordingly.
Perform varying office duties, including but not limited to, answering multi-line phones, faxing, copying, e-mailing.
Prioritize schedule to effectively handle evolving issues and tasks.
Communicate the needs of the consumer to providing agencies in a timely and effective manner.
Respond to all consumer needs, complaints, issues, etc...
Other duties as assigned by the Administrator and/or Service Coordinator or as needed for the implementation of new OLTL codes, systems and updates.
The preceding list of essential functions is not exhaustive and may be supplemented. KNOWLEDGE, ABILITIES, AND SKILLS Ability to read and understand regulatory standards. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to apply common sense to deal with problems involving a variety of factors in normal situations. Ability to communicate effectively with staff, consumers, consumer?s family members, members of the community, and with representatives of the Office of Long Term Living or other governmental agencies. Has basic computer experience and using word processing software and email.
Required education:
To provide service coordination services, a service coordinator shall meet either of the following:
(1) Have a bachelor's degree including or supplemented by at least 12 college-level credit hours in sociology, social welfare, psychology, gerontology or another behavioral science.
(2) A combination of experience and training which adds up to 4 years of experience, and education which includes at least 12 semester hours of college-level courses in sociology, social work, social welfare, psychology, gerontology or other social science.
(i) Experience includes:
(A) Coordinating assigned services as part of an individual's treatment plans.
(B) Teaching individuals living skills.
(C) Aiding in therapeutic activities.
(D) Providing socialization opportunities for individuals.
(ii) Experience does not include:
(A) Providing hands-on personal care for people with disabilities or individuals over 60 years of age.
(B) Maintenance of the individual's home, room or environment.
(C) Aiding in adapting the physical facilities of the individual's home.
