POSITION SUMMARY
Title: Case Manager
Reports to: Clinic Administrator
Status: Exempt
POSITION SUMMARY
Position involves coordination of patient care and community services to patients in an integrated care model. Position functions in collaboration with referring practitioner (primary case manager) and is clinic based.
PRIMARY ACCOUNTABILITIES

Timely referral coordination and documentation
Management of Quality Improvement Project Tracking
Satisfactory completion of coordinator functions related to case management
Timely management and tracking of Behavioral Health patient appointments

Demonstrate excellence in management skills.
Relationships
Develop and maintain favorable internal and external relationships with multiple strategic partners including co-workers, managers, providers and support staff.

Uphold and insure compliance and attention to all corporate policies and procedures as well as the overall mission and values of the organization.
PRIMARY TASKS AND DUTIES
Works in conjunction with interdisciplinary team; reports to behavioral health.
Provides functional evaluation and consultation to identify psychosocial barriers and works to assist patient in resolving those barriers.
Referral and brief case management.
Insurance consultation.
Coordination and implementation of quality improvement projects where appropriate.
Understand and support the mission, goals and agency wide standards of KCHC.
Develop and maintain cooperative relationships with KCHC staff and community agencies.
SECONDARY TASKS AND DUTIES
Perform other related duties as required, which include but are not limited to:
Project planning and coordination
Perform other duties as assigned.

POSITION REQUIREMENTS
Education
Bachelors Degree in a Human Service related field (Social Work, Sociology, Criminal Justice, Psychology)
Experience
Bilingual (Spanish) preferred
