RN, LPN, or MSW - Care Manager Job Opening in Richmond, Kentucky - Are you currently or have previously worked as a Care Manager for a Healthcare or Insurance Company? If so, we need you! Experienced RNs, LPNs, or Social Workers are needed for part time assignments providing Case Management Services to eligible home care clients. Duties include conducting home visits and providing assessments where the elder resides, designing an appropriate care plan for the elderly/disabled client and assisting with resources.
