NNI currently has an opening for an Occupational Therapist (OT) to provide intermittent home health visits for our Prescott Valley and Quad city area patients.
Occupational Therapist (OT) Requirements:
Licensed in Occupational Therapy
At least 1 year of experience as an OT
All employees must pass a pre-employment drug test
Be able to obtain a Level One Fingerprint Clearance Card.
Occupational Therapist (OT) Responsibilities include but are not limited to:
Develops (with the help of patient and caregiver) a plan of treatment (POT) that is reasonable, necessary and appropriate to the needs identified in the home.
Determines if additional visits will be required to accomplish the goals established on the POT.
Documents all therapy interventions and teaching, patient response to interventions and progress toward goals. Completes all required documentation (including therapy functional assessments, OASIS, discharge summaries) in the EMR in a timely manner, per NNI policy/procedure.
Presents a positive, professional image in appearance and attitude to patients/caregivers and fellow NNI staff members.
Provides updated information to agency staff through in-service education or as consulted.
