PRN Nurse Practitioner- Jefferson County Job Opening in Birmingham, Alabama - Complex Care Solutions is an industry leader in Health Plan Member Assessment and Care Management. We partner with Health Plans in 23 different states across the US. We are contracted by these health plans to provide in home health assessments on their members. This allows the members to stay safe and healthy at home and prevent admissions to healthcare facilities.
We are currently looking for a Nurse Practitioner to help complete these home health assessments to capture a comprehensive picture of the patient?s overall health. This Nurse Practitioner will not administer treatment or prescribe any medication. The purpose of the visit is to confirm prior diagnosis? and report any news ones or any other risk factors they may notice.
Requirements:
Current NP State license in hand
NPI #
Board Certification Preferred
Experience in: Family, Geriatric (Gerontology), Psychiatric, Acute Home, Hospice, Homecare
Reliable vehicle
Experience with EMRs
