A dedicated RN who has extensive experience in Home Health and now wants to contribute to the growth of an exciting new ?care first? model in healthcare. This RN will be fundamental in establishing a reputation for superior quality of care and customer service and growing a clinical staff of excellence.
Job Summary: The Home Health Registered Nurse Case Manager assesses patients for skilled home health needs, develops and initiates a plan of care with the physician approval and provides nursing care, ordered by the physician to patients in their place of residence. The Home Health Registered Nurse continually re-evaluates the patients nursing needs.

Qualifications:
Licensed, Registered Nurse in the State of California
Experience: minimum of 2 years experience in home health
Social Security Card and/or proof of eligibility to work in U.S.
Valid CA Driver?s license and auto insurance
Current CPR
Current TB/Physical
Background Clearance
Some supervisory experience preferred
EMR and Oasis documentation experience preferred
