We want you to bring value, and feel valued every day! We are a non-profit community health center that provides healthcare to medically underserved populations. HOPES programs address conditions that often challenge people with stigma, isolation, addiction, mental health, hunger and homelessness.
Job Must Haves:
A dynamic, compassionate, experienced case manager to coordinate care for underserved populations; designated for primary care
MUST have experience with resources in our community, with at least 3 years? experience in case management or related field?show us what you got!
Extensive knowledge of local community resources and share in HOPES mission and vision; see below for our website where you can get acquainted with us!

Bachelor?s degree in Public Health or Social Work preferred
Experience in developing succinct and practical care plans with high acuity patients
Experience in utilizing care plans in an electronic health record a plus; and we like knowledge of HIPAA!
Strong data entry skills and follow through?if it?s not documented, it didn?t happen!
Ability to identify and make referrals to other social service agencies in Northern Nevada
Along with empathy and a STRONG desire to help people succeed, especially critical for this position is the ability to effectively work in a collaborative environment with other case managers and members of the client?s treatment team (doctor, nurse, therapist, etc) to ensure that the client is receiving the most comprehensive care possible!
