Novus Healthcare is seeking Nurse Practitioners to conduct in home and clinic visits for patients in Greensboro, North Carolina. Activity-based compensation and flexible hours are available. Novus Healthcare provides our practicioners with the training and tools to do more of what you love-working independently to see patients and treat complex medical conditions.
Position Summary
This position is responsible for the assessment and coordination of patient healthcare needs through an Annual Wellness Visit (AWV) and provides expert consultation and guidance to clinic staff regarding health issues of their patients. The Nurse Practitioner will complete AWV assessments in the patients' homes or at their primary care clinic.
Responsibilities
Establish supportive, motivational and therapeutic relationships with patients ensuring an atmosphere of safety and security.
Complete an AWV assessment for all patients seen in their home or in the clinic.
Maintain a collaborative relationship with the team and clinic staff.
Complete required trainings, both upon hire and annually.
Other duties as assigned.
Knowledge, Skills and Abilities
Ability to intervene appropriately in crisis prevention and de-escalation when necessary.
Ability to communicate effectively orally, in writing and on both an interpersonal and group level.
Excellent interpersonal skills working with people from different backgrounds and cultures.
Ability to maintain confidentiality of client information and appropriate boundaries.
Qualifications Education and Experience
Licensed as a Nurse Practitioner in North Carolina and in good standing
Minimum of one year working with the elderly preferred
Current CPR certification
Other Requirements
Interest and ability to work independently as an independent contractor with little supervision
Willingness to travel to patients' homes within the surrounding area to conduct patient visits
A valid drivers license and proof of liability insurance
Familiarity and comfort with Electronic Health Records and Microsoft Office
