JOB SUMMARY: Qualified candidate who represents the agency to its patients/patient care givers and family members as well as other members of the community. Responsible for assisting in patient , quality management and data collection.
DUTIES AND RESPONSIBILITY:
Responsible for his/her patients assigned to them, answer non-clinical questions while educating the patient in home health care and assisting in the transition of care from facility based to home based as necessary.
Reviews the admission process and packets with the patient and acquiring all required signatures for admission.
Responsible for making follow up phone calls to assess patient care and patient satisfaction
Responsible for tracking and analyzing data to help identify areas for improvement in care quality and delivery.
Attends meetings including but not limited to staff meetings, in-services, clinical team meetings, educational classes, office meetings and/or agency meetings.
Meets with agency field and office staff regularly to communicate patient needs and find answers to patient/patient care giver/family questions.
Abides by all Medicare/State home health Standards and agency policies and maintains the highest standard of integrity at all times.
Identifies or confirms the patient's primary care physician and helping to set up a follow up appointment with physician
Shows compassion and care for patients and puts their needs first at all times.
Able to solve problems through phone communication with all departments of the agency and the patient.
To effectively communicate with customers, patients and the Medical community.
When clinical questions are asked, the care transition representative directs them to the appropriate clinician
Represents and interprets the Agency's functions to the patient/patient care giver/family, the public and other organizations in a positive and professional manner.
Performs related and other duties as needed.
Qualified candidate with a Bachelors or Associate degree in a related field and with a proven record in customer service. Medical office experience preferred.
Able to communicate effectively in English, both verbally and in writing. Additional languages an asset. * Travel >50%
