Primary Duties:
Responsible for the development of the Stroke Program, participates in the design of the internal/external marketing of the program, coordinates education for nurses and ancillary staff involved in the care of stroke patient.
Develop and maintain community and staff education programs. Prepare educational materials for outreach events, stroke screenings and lectures at community outreach sessions twice yearly (minimum).
In collaboration with the Stroke Center Director and Stroke Center Administration, nursing and physician staff, develops, maintains and monitors stroke clinical practice guidelines (CPG), order sets and clinical pathways utilizing the American Heart Stroke Association, Brain Attack Coalition (BAC) and National Institute of Neurological Disorders and Stroke (NINDS) guidelines.
Facilitates development of education program for staff-orientation, maintains yearly competencies in stroke evaluation tool proficiencies and facilitates development and implementation of education program for Stroke patients and significant others.
Develop QA initiatives to monitor various aspects of ongoing stroke center PI process and completes data abstraction for all stroke patients according to the guidelines established by the TJC and/or the Brain Attack Coalition. Incorporates data and information to continuously improve care and practice to enhance outcomes.
Data abstraction of Stroke Performance Measures as per guidelines established by the TJC and the NJDHSS, utilizing the New Jersey Acute Stroke Registrar Tool.
Assessment and care management of the stroke population under the supervision of the Stroke Center Medical Director participating in multidisciplinary patient rounds, performing NIHSS and Swallow Screen as required.
Responds as a member of the Acute Stroke Team.
Coordinates care of Stroke patients with the various Stroke Team members: physicians, therapists, case managers/social workers, clinical nutritionists, respiratory therapists, radiology, and lab technicians.
Secondary Duties of Responsibility:
Performs extensive medical record reviews and concurrent chart data abstraction on all regulatory required clinical indicators for assigned patient group, as required by TJC, CMS and any other any other accredit ting, state and federal agencies.
Select and collect appropriate data from patient charts relevant to the Core Measures Set.
Monitors the appropriateness of patient care based on evidence-based outcomes and clinical guidelines.
Maintains confidential communication with physicians and those clinical disciplines directly involved in the care of the patient, and aligns core measure requirements as necessary.
Maintains knowledge of Core Measures through self-development and keeps current on all changes related to Core Measure requirements.
Trains and educates others in the area of Core Measurement requirements, as it pertains to Quality / Performance Improvement.
Participates in Performance Improvement Teams, Root Cause Analysis, Failure Mode Effective Analysis and any other committees / task force as appropriate.
Chairs committees / task forces as assigned.
EDUCATION:
Graduate of an accredited school of Nursing in the state of New Jersey.
Masters degree in Nursing preferred but will consider candidates pursuing their masters
EXPERIENCE:
Possess clinical expertise in neurological disorders, especially stroke
High degree of accuracy and efficiency in record keeping and clinical functions
Excellent organizational and critical thinking skills

BLS Heart Saver/Health Care Provider Certification
