Part Time HIV Health Educator Job Opening in Portsmouth, Virginia - HIV Health Educator will provide community based HIV/AIDS case management and health education services that focus on People Living with HIV/AIDS (PLWHA), that have either fallen out of or are sporadically engaged in HIV care and treatment. This dynamic model of case management and health education services will actively address the multiple needs of PLWHA and remove the barriers that prevent retention in care and treatment. The program will service people living with HIV/AIDS in Portsmouth, VA. This case management model serves as a bridge to help consumers learn to navigate the health care and support system and gain the knowledge and skills necessary to effectively self-manage their care. It also helps support consumer entry into the full continuum of care that is available.
Job Requirements:
Client recruitment and engagement
Must be flexible to work days and evenings
Screening and assessment: Completing risk assessments for HIV, STD, sexual behaviors and substance abuse
Participate in program outreach as designated by program needs prescribed in the ?Scope of Work?
Conduct one on one ?risk reduction counseling? sessions with participants
Facilitate group meetings including; standing, presenting, and completing required administrative work
Provide information and referrals for HIV/STD and other services
Development of outreach sites and group locations
Assist with the completion of monthly administrative responsibilities
Attend community meetings, community events and trainings as designated
Computer literacy: Word processing and file management
Must be able to work effectively and professionally as a team member
Must be able to recognize and accept responsibility for all parts of the job
Ability to sit, stand, bend, walk, as required, for carrying out duties of position
Have experience and sensitivity in working with diverse organizations and constituencies including the LGBTQ
Others duties as assigned by supervisor
OTHER REQUIREMENTS:
Must have at lease two years experience working in the HIV/AIDS field
Knowledge about, understanding of, and ability to work closely with, persons with HIV/AIDS and related issues.
Two to three years of experience facilitating group and individual health education services.
Effective communication skills and culturally and linguistically competent.
