RN / LMSW (HHA or Hospice Experience Required) Job Opening in Woodbridge, Virginia - NurseTrek is now hiring an experienced Registered Nurse, or Licensed Master Social Worker. There are immediate openings to do on-going case management services for healthcare members in the Woodbridge, VA and surrounding regions:
Qualifications & Requirements:
Must have an active Virginia license with a minimum 1 year in-home case management experience
Must be able to work independently
Must be willing to travel to clients homes within a 40-45 mile radius of your location
Must have experience traveling to a client's residence
Home Computer with Internet and Cell Phone Required (Computer and Internet Proficiency)
Required Company Specific Training Provided
Experience With Computerized Charting preferred
Must pass drug screening and background check
Must be able and willing to complete required training and begin working within 5 days of job offer
This job will have the following responsibilities:
Coordination and scheduling of client visits
Completion of paperwork in a timely manner
Conducting in-home visits
Member Assessment, Identifying needs, Education on Disease process, Safety, as well as communication with Primary Care Physician
Assessment of needs for Community Resources
Caregiver Education
Monitoring of member and referrals to appropriate resources
Additional duties as needed
