Post Acute Network (PAN) Representative Job Opening in Richmond, Virginia - Remedy Partners is seeking a Post Acute Network (PAN) Representative to join our growing Tribe. The PAN Representative is a client-facing manager for the Bundled Payment program we are building in association with a partnering hospital or health system in your region.

The PAN Representative is responsible for approximately 20-30 Post Acute Care (PAC) facilities covering, on average, 100-300 cases. You will report directly to our Market Manager and learn first hand how to impact our fast-paced, innovative culture. You will work remotely from home, traveling 80% of the time within your region (primarily ground travel).

We are looking for someone with:
Bachelor?s degree
3+ years experience the healthcare business and/or nursing (or other clinical roles) in assigned region
Personal Drive to succeed and an entrepreneurial spirit to overcome myriad challenges in a changing healthcare environment
Strong critical thinking needed to analyze medical data and reports
Understanding of Care Transition Models (Naylor, Coleman, Boost, etc..) is a plus
Patient assessment and evaluation experience, ability to exercise independent judgment
Case management and discharge planning experience are preferred but not mandatory
Effective written and verbal communication skills and the ability to communicate with all levels of individuals from both our organization and our partners
Compassion and trust - know how to lead with kindness and gain trust / buy-in
Proficiency in Microsoft Office (Windows, Outlook, Excel) with the ability to learn new applications.
What will you do?
Coordinate and lead bi-weekly meetings with PAC faculty at each facility to report outcomes and suggest improvements
Establish and achieve quality improvement and cost reduction targets for network of facilities
Identify high priority care management changes to improve performance and efficiency of facilities within local network
Identify outlier cases (outside of Remedy guidelines) for intervention by clinical staff
Assure Episodic case flow and develop performance relative to BPCI length of stay goals
Responsible for rounding at nursing homes and developing Post-Acute network within local region of facilities
Communicate with the Remedy Partners Care Coordination Call Center regarding various patient needs
Evaluate performance of individuals within facility to maintain consistent Remedy culture quality and standards
Resolve administrative and operational issues as needed
Provide feedback to Manager and Analytics team to improve care reporting
