Case Manager Job Opening in Little Rock, Arkansas - Case Manager, Supports Coordination
The Case Manager, Coordinates and providing care that is safe, timely, effective, efficient, equitable, and client-centered. Handling case assignments, draft service plans, review case progress and determine case closure. Helping clients achieve wellness and autonomy.
The Case Manager also, remotely manages a team of supports coordinators (field counselors) who meet with participants in-home to train and monitor them as they establish and manage their own health care through the Self Direction with Service Budget (SDSB) medical delivery system. Case Managers are responsible for ensuring all SDSB program and contract directives are met while delivering person-centered instruction and support.
Duties and Responsibilities
Ensure SDSB contract requirements and program deliverables are met.
Ensure quality assurance through documentation review, phones calls with participants, reviewing daily reports and making in-home visits to participants.
Facilitate multiple care aspects (case coordination, information sharing, etc)
Develop effective working relations and cooperate with medical team throughout the entire case management process
Arrange abeyances with social services, health and governmental agencies
Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
Facilitate training via email, teleconference, video conference and in person.
Attend frequent trainings and meetings via email, teleconference, video conference and in person at corporate headquarters in Little Rock, Ar
Maintain availability full-time for contact with counselors, clients and corporate headquarters.
Review and submit reports regarding case management and SDSB supports coordination
Coach, correct and evaluate supports coordinators within the region
Record cases information, complete accurately all necessary forms and produce statistical reports
Promote quality and cost-effective interventions and outcomes
Assess and address motivational and psychosocial issues
Adhere to professional standards as outlined by protocols, rules and regulations
Other Essential Traits
Must reside within the region established by Palco.
Must possess professional verbal and written communications skills.
Must possess string organizational skills, problem-solving skills and the ability to make decisions autonomously.
Must possess proficient skills with Microsoft Office, particularly Excel, Microsoft Outlook and Internet.
Must be able to electronically document records to a web-based chart.
Must have a driver?s license and a dependable mode of transportation.
Must possess a Social Security number or paperwork documenting legal residence in the United States.
Must be able to pass a criminal background check, maltreatment check and drug screen.
Problem solving skills and ability to multi-task

Education and Experience Requirements
Proven working experience in case management, including as a nurse, medical, mental ' health care manager or a related job
Excellent knowledge of case management principles, healthcare management and reimbursement
Previous experience with psychological aspects of care
Familiarity with professional and technical emerging knowledge
Problem solving skills and ability to multi-task
Compassionate with teamwork skills
Current license, certification or registration
BS degree in related healthcare field
