LPC OR PLPC for Intensive Outpatient Program Job Opening in Mansfield, Louisiana - Counselor (LPC/PLPC)
Responsibilities include:
with personal computers and Microsoft software applications. Ability to work in a structured, fast-paced environment.
to effectively collaborate with members of the therapy team, Therapy Supervisor, Program Director, and Medical Director.
to provide quality patient care, is skilled in and conducts group, individual, and family therapy using appropriate counseling techniques.
to be creative, well organized, flexible, and manage time efficiently. Possess solid communication skills, both verbal and written.
to work well with other healthcare professionals in the community.
and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient's aftercare needs.
and assesses family or community resources such as, group homes, or mental health practitioners to meet patients' aftercare needs.
with referral sources and other appropriate parties regarding patients' progress.
and implements a plan of treatment, in conjunction with members of multi-disciplinary team.
didactic and other appropriate patient groups. Medicare and Private Insurance compliant documentation.
and implements patients' discharge.
Attends and participates in department and other program or clinical and administrative meetings.
