Physical Therapist Assistant Job Opening in Lake Charles, Louisiana - At the Guardian Healthcare and Rehabilitation we provide short term Rehabilitation program consisting of Physical, Occupational and Speech/Language and Dysphasia services. Services are provided by licensed physical, occupational and therapist from 5-7 days a week. The Guardian Healthcare and Rehabilitation facility has a unique association with The Gardens, which is an assistive living facility that is on the same campus as The Guardian Healthcare and Rehabilitation facility. We are also unique in that we are the only facility in Lake Charles, Louisiana that also provides outpatient therapy and long-term care and skilled nursing that focuses on geriatric care.
OUR THERAPIES
PHYSICAL THERAPY
Physical Therapy is provided to maximize or restore a patient?s ability to function as independently as possible by utilizing various therapeutic techniques. Our facility provides therapy services on a one-one basis, based on patients? needs, from 5-7 days a week. We are the only facility in Lake Charles that provides body weight treadmill training (BWSTT).
OCCUPATIONAL THERAPY
Occupational Therapy addresses activities of daily living (ADL?s) such as: bathing, dressing, and feeding and light home management task. Our therapists utilize a functional kitchen and laundry area where they train patients for safe and functional return to their previous home environment with the goal of maximizing or restoring independence.
SPEECH THERAPY
Our therapists works with patients who are experiencing difficulties with swallowing, language, speech and / or cognition. Our Speech Therapy team use science-based solutions for highly personalized protocols for patients with traumatic brain injury, stroke, aphasia and cognitive decline. They also manage individual with dysphasia caused by various underlying impairments for management and targeting specific functions or structure of swallowing. Treatment of dysphasia may include restoration of swallowing function, modification of diet and / or a combination of these two approaches. The goal is to establish the patient?s maximum levels of independence and safely in those areas.
THE TEAM APPROACH
To meet the individual needs of each patient, their rehabilitative program is highly individualized and overseen by the attending physician and provided skilled nursing services.
The duration of therapy services per day is specific to the individual patient?s needs. Determining factors include, however may not be limited to, patient?s activity tolerance, medical condition, goals to be achieved and the number of disciplines involved in the care
Therapy sessions are conducted on a 1:1 basis with patient?s basis with patient?s resulting in more specialized and focused treatment and aiding in patient?s recovery.
Over 98% of our short-term patients go home or to a more independent living environment, such as The Gardens.
Teaching sessions are provided to family members by appointment to assist in safe discharge for the patient. A home evaluation is provided to each patient.

Our Transitional Care Unit
Workings with our therapy team and while under physician care and being provided skilled nursing, allow patients to be equipped with a safe and swift return to their home environment. Our team of trained professionals will provide a thorough and careful assessment and planning a will determine if our beloved clients are ready to go home independently or might need some assistance after therapy discharge. To ensure this safe transition home, we at The Guardian, will provide our patients with an opportunity to spend 3 days during daytime hours at The Gardens ?in our Transitional Care Unit. This furnished apartment with a bedroom and kitchen are provided during the assessment period allowing for a realistic simulation of a home environment ensuring the patients and their families the confidence and assurance for a safe transition home.
