Registered Nurse - Weekend On-Call RN Job Opening in Darlington, South carolina - Description: Hospice Care of South Carolina is a place where you can impact the lives of others by helping our patients to live life to the fullest with dignity and comfort. We are the largest provider of hospice services in the state of South Carolina. We are dedicated to patient care, quality service, and employee satisfaction. We are currently seeking a compassionate and dedicated professional to join our team as a Full Time Weekend On-Call RN to cover Darlington County.
*This position involves weekend on-call every weekend from Friday 5pm-Monday 8am.
Duties and Responsibilities:
Provides direct and indirect care within the scope of the RN as outlined in the SC Nurse Practice Act, established policies and procedures, standards of care and other regulatory and licensing agencies.
Conducts patient visits as scheduled by the Patient Care Coordinator.
Reports changes in patient condition and communicates other appropriate patient information to Patient Care Coordinator or other members of the Interdisciplinary Group.
Completes documentation per HCSC?s policy requirements.
Participates in the comprehensive assessment of each patient as assigned.
Determines the patient is hospice appropriate, documents to patient?s terminality, establishes and maintains individualized plan of care.
Attends Interdisciplinary Group meetings, staff meetings, and committee meetings as assigned.
Provides information to family as to the health status of the patient routinely, and in times of significant change in condition.
Assesses patient?s individual health status using accepted tools and documents to determine hospice eligibility.
Seeks and obtains two (2) physician?s certification of terminal illness.
Obtains the Election Statement from the patient/family for Hospice benefits in collaboration with the Community Relations Director.
Completes the initial nursing assessment per HCSC?s policy requirements.
Completes and implements the Plan of Care per HCSC?s policy requirements.
Collaborates with the appropriate discipline on the comprehensive assessment of the patient.
Performs supervised HHA visits per organizational policy.
May be required to serve in HHA capacity.
May assist with nursing orientation in the preceptor role.
On call responsibility in conjunction with other nursing staff through call rotation schedule and in accordance with response time requirements.
Actively seeks ways to develop professionally by pursuing expertise in hospice nursing care, continuing education and mandatory in-services.
May be assigned for crisis care to a specific patient under the direction of the Patient Care Coordinator or Regional Nursing Director.
Performs other duties and responsibilities as assigned by the Patient Care Coordinator.
Physical Conditions:
Must be in good physical and mental health, free from infectious diseases
Must be able to work weekend hours.
Lifting ? may be required to lift patients, medical equipment, and supplies, with weights up to 50 pounds. May move objects from a lower to higher position or along horizontal level from position to position.
Vision/Hearing ? must possess the power to perceive sound with or without correction. Ability to make fine discrimination in sound. Eyesight needed, or prosthetics used to enable adequate sight to meet requirements.
Mobility ? must have mobility skills as required with standing, walking, stooping, bending, lifting, and reaching. May have prolonged periods of sitting and driving.
Ability to maintain habits of personal cleanliness and safe work practices.
Ability to perform duties with regards to infection control guidelines. Ability to instruct others within the Interdisciplinary Group of proper and adequate infection control guidelines
Other ? may be subject to falls, infectious diseases, or odors throughout the work-day. Must have the ability to perceive through physical contact, especially the fingertips.
Minimum Qualifications ():
Licensed as a Registered Nurse, in good standing in South Carolina
Current BLS
Minimum of one (1) year of clinical RN experience or three (3) years of prior LPN experience
Experience specific to patient population served is preferred, hospice and/or home experience is desirable
Clinical Judgment: Demonstrates critical thinking, analysis and clinical judgment in the care of hospice patients and families through use of the nursing process to address the physical, emotional, and psychosocial, and spiritual needs of the patients and families.
Professionalism: Demonstrates knowledge, attitudes, behaviors, and skills that are consistent with the professional standards, code of ethics, and scope of practice for Hospice nursing.
Communication: Demonstrates the use of effective verbal, non-verbal and written communication with patients, families and members of the healthcare team and community in order to therapeutically address and accurately convey the hospice/palliative care needs of patients and families.
Cultural Competence: Demonstrates cultural competence by respecting and honoring the unique diversity and characteristics of patients, families and colleagues hospice care.
Collaboration: Actively promotes dialogue with patients and families, the healthcare team and community to address and plan for issues related to hospice/palliative care.

Must have a reliable transportation, valid driver?s license and auto insurance
