Patient Advocate Job Opening in Cranston, Rhode island - JOB SUMMARY :
Ensure and provide support to care management team, and program management. Assist the care management team (nurse care managers, pharmacists and patient care coordinators) with a focus on excellent customer service. Promote wellness, increase patient knowledge, and improved patient outcomes in a patient centered approach.
ESSENTIAL JOB FUNCTIONS :
General Responsibilities:
Work collaboratively with office practice staff, and physicians
Work collaboratively with RIPCPC staff including and not limited to, nurse care managers (NCM), pharmacists (RPh), patient care coordinators (PCC), quality improvement specialists (QIS) team members
Active participation in team meetings
Printing and mailing of patient education information and other administrative support services
Complete documentation of outreach following work flow based on patient type
Provide Care Management Support:
Provide outreach to patients for follow-up to discharges from emergency department visits or inpatient hospitalizations or skilled nursing home stays
Triage patient to appropriate care team member (NCM, RPh, PCC)
Complete tasks assigned by care management team for patient assistance and follow-up
Provide Pharmacy Team Support:
Provide outreach to patient for medication adherence support and monitoring
Assist in assessment and completion of patient assistance forms for patients
Complete tasks assigned by pharmacy team for patient assistance and follow-up
Provide Patient Wellness Support:
Provide outreach and support to patients assigned to regular check in calls
Triage patients to appropriate care team members if needed for additional assistance and support
QUALIFICATIONS:
Minimum Education and Experience:
High School Graduate
3-5 year work experience in health care (medical assistant, pharmacy or related work)
Experience with Microsoft Office ? Excel, PowerPoint, Word
Effective oral and written communication skills
Skilled in patient engagement strategies and techniques
Skilled in telephonic interactions
Experience with customer service
Highly self-motivated
Flexibility and ability to shift work prioritization based on need
Good understanding of healthcare system and how assist patients in navigation
Preferred Education, Additional Qualifications and Experience:
EPIC Trained
Bilingual ? Spanish preferred
Experience working in teams
Certified Pharmacy Technician
Past experiences with direct patient contact
Specific knowledge and training in prescription and OTC medications
