PACE of RI has an exciting opportunity for an experience Home Health Care Manager
Position Overview:
The Home Care Operations Manager position reports to the Vice President of Clinical Services.
This position manages all scheduling activities related to services provided in the home and internal/external clinics for PACE clients; provides a focal point for participants; facilitates the coordination of services to PACE clients in a timely manner; acts as a first contact or liaison to home care vendors, health care professionals and internal case management staff.
Primary Responsibilities:
Manages the day to day operations of the home care services including participant intake, staff assignment, scheduling, and supply utilization.
Expedites communication with participants, families and others involved in the delivery of services to the participant.
Completes all documentation and record keeping related to the provision of Home Health Care skilled nursing according to established agency procedures.
Recruits Home Care staff, provides orientation and training in all areas.
Supervises home care staff in accordance with the organization's policies and applicable laws.
Participates in planning, implementation, evaluation, and quality initiatives of agency programs.
Attends and participates in agency meetings and mandatory education programs and attends relevant continuing education programs.
Qualifications:
Associate or Bachelor Degree preferred. In lieu of degree, equivalent experience required.
Minimum of 3 experience in home care supervisory or management role
Minimum of 2 years experience working with frail or elderly population
Advanced knowledge of Electronic Health Record (EHR), practice management and scheduling software
Excellent communication and presentation skills
Valid RI Driver License and access to reliable transportation; travel throughout RI required
