RN - CASE MANAGER FOR HOME HEALTH CARE COMPANY Job Opening in minneapolis, Minnesota - RN - CASE MANAGER FOR HOME HEALTH CARE COMPANY Visiting Angels Living Assistance Services is a Class-A licensed private home care agency that is dedicated to providing safe, caring, and understanding companionship, personal care, and home support services to adults of all ages in the comfort of their own home. Visiting Angels of White Bear Lake is currently seeking a reliable, dedicated, compassionate, and innovative Registered Nurse to provide a thorough assesment of clients seeking services through our agency in the East and West Metro. The RN will be responsible for developing appropriate individualized care plans, collaborationg with the client's family and/or primary physician as needed, and assessing and re-assessing the client's needs including: physical, psychosocial, and environmental needs. Acting as care manager for assigned individuals /families, the RN will direct and supervise Certified Nursing Assistans and Home Health Aides in regard to the client, and initiate referrals as necessary. ***MINIMUM QUALIFICATIONS*** Current State Registered Nurse license and CPR certification. Graduate of an accredited nursing program. One year of continuous nursing experiences or equivalent. Ability to meet bonding criteria. Valid driver's license, available personal vehicle, and proof of auto insurance. Excellent verbal, written, and organizational skills. Ability to interest in being part of growing the business. Minimum one year Home Health Care experience. Extensive assesment skills. Working knowledge of Class A license requirements.
