Virginia Garcia Memorial Health Center is seeking a full time Clinical Workflow Specialist (?CWS?) who frames the work, identifies scope items and objectives, and puts forth recommendations based on review of current VG standard work and documented best practices from industry leaders. In collaboration with the Medical Leadership Team, this position is responsible for developing, implementing and evaluating improved work processes related to all clinical aspects of Primary Care.
Essential duties and responsibilities:
Collaborate with outside organizations to understand industry best practices.
Deliver Team Based Care best practices in collaboration with project leadership and care teams.
Develop and document proposed workflows, policies, procedures and tools to be tested, with a focus on efficiency of team-based care and shared use of EPIC.
Work closely with Project Management to ensure delivery of practice improvement designs and work processes that are patient centered, and that improve quality of care, reduce costs, and improve staff satisfaction.
Support Beta Teams and CI teams throughout PDSA and Lean improvement processes.
Collaborate with Training staff, Site Specialists, and Data Analysts on creation of tools and training materials.
Collaborate with CPMs/Site Leadership throughout project cycle to prepare for spread of solutions across clinics.
Acts as subject matter expert in close collaboration with project and clinical leadership.
Participate in the design, testing and evaluation of potential population based programs.
In collaboration with Medical and site leadership, communicate proposed designs and findings to leadership and staff.
Perform other duties as assigned.
Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Education and/or Experience:
Three to five years? recent experience as a Registered Nurse; active RN license preferred.
Experience leading teams through design process.
Experience in a community health center setting preferred; broad primary care experience required.
Experienced EPIC user.
Experience as a team coach.
Experience in building and sustaining multi-stakeholder engagement and collaboration.
Trained and experienced with Lean or similar process improvement methodology.
