CPST - Adult ACT Team Leader Job Opening in Athens, Ohio - Hopewell Health Centers, Inc. Behavioral Healthcare in Athens has an opening for a full-time ACPST ? ACT Team Leader.
Position Duties:
CPST service provider delivers quality, person and family-centered, medically necessary, rehabilitative, Community Psychiatric Supportive Treatment interventions including but not limited to: assessment of needs, skills , crisis management and intervention, advocacy, education/training, eliminating barriers, , and monitoring.
Adult CPST Team Leader Duties:
- Team Leader Duties
Ensure that all cases are seen and followed in a timely manner in accordance with clinic access guidelines/clinical need.
Assign cases/reassigns cases.
Facilitates team meetings on a regular basis.
Reviews cases on weekly basis with staff and as needed with Administrative Supervisor.
Provides clinical support as needed.
Interfaces with Adult/Child St/430 W. Union St/Blue Line Dr./Sami Housing and any contract partners to insure that client needs are being met.
Liaisons with community partners: GP, AMHA, 317 Bd, Athens Homeless Coalition, ISS, HRS, Medical community, SSA, JFS, food banks and etc.
- Clinical oversight of ACPST Team, including but not limited to:
Complete yearly clinical evaluations.
Directs/provides emergency backup to ACPST providers.
Assures coverage, approves vacation, signs time sheets in the absence of the ACPST director.
- Provides Program supervision to all ACPST team members on a weekly/regular basis.
Is available for daily case consultation.
Reviews daily progress notes for accuracy/treatment relevance and the golden thread of treatment.
Stays abreast of daily census in ABH/Respite/housing units.
Reviews applications for housing assistance in the form of vouchers, special programs, start-up funding, loans and etc.
Reviews ISPs/DAS updates/reviews for accuracy and appropriateness before forwarding documents to the ACPST director for clinical review/acceptance.
Receives initial complaints regarding client care .
- Is a member of the HURC committee, transition team, combined ABH/HHC meeting, SAMI housing and any other internal committee as designated by the ACPST director.
- Collaborates with ACPST director in clinical trainings.
- Collaborates with ACPST director in developing clinical portions of agency materials.
- Collaborate with ACPST director to ensure quality of care.
- Act as back up for other team leaders, as needed.
- In collaboration with person served and/or family/legal guardian, develops an individualized service plan (ISP) for each person in the program to achieve behavior change, increased quality of life, and the best outcomes possible for the people we serve. Generates a current, legible integrated clinical record (ICR), in which documentation reflects a clear clinical picture for assigned person served.
- Maintains minimum acceptable billable expectation as designated by agency UR sheet. Follows billing procedures as required by agency policy.
- Completes correctly and submits in a timely manner per agency policy documentation, billing, time sheets, mileage, and, as required by supervisor(s) work and appointment schedules.
Attends as scheduled and participates in supervision, team meetings, professional development, First Aid/CPR training and serves on assigned agency committees, as appropriate.
- Informs supervisor(s), client rights officer, or human resource director, as appropriate, of any complaints from persons served, families, members of the community, or workforce members; suspected workforce member abuse/harassment towards persons served or their family/significant others or other workforce members.
- Abides by agency code of conduct, policy and procedures and mission statement (e.g., business practices, clinical, personnel, and plan manuals) at all times on agency property or while conducting agency business.
- Performs other related duties as assigned by supervisor(s), program director, clinic director, and agency administration.
Minimum Requirements : LSW, RN or PC; Demonstrates the skills and ability to communicate and work with persons receiving service, their families, community service providers, and team members in the assessment, problem solving, and delivery of rehabilitative services; the ability to effectively communicate verbally and in written form; A valid driver?s license, automobile insurance, good driving record, and suitable vehicle for transportation of persons served; and the ability to assist persons into and out of a vehicle.
Preferred Qualifications: Bachelor Degree in Social Work or closely related field and a LSW or LPC .
