MDS Coordinator RN Job Opening in Fairburn, Georgia - MDS Coordinator
We are currently seeking an experienced MDS Coordinator, who has a willingness to work with a team, a desire to improve resident care, and the ability to coordinate resident centered care, to join our team.
Job Duties include:
This position is responsible for ensuring appropriate reimbursement of Medicare and/or Medicaid Residents through the Resident Assessment Instrument (RAI) process.
Assist in the management of quality Resident care on a continuing basis in accordance with federal and state standards and as may be directed by the Administrator or Director of Nursing.
Schedule, complete, and transmit the assessments according to the established RAI timeline
Being an integral part of the care planning process
Develop a resident-centered care plan that encompasses the resident?s voice and speaks the needs of the resident
Schedule care plan meetings for the resident and/or legal healthcare representative along with the Interdisciplinary Team
Participates in the assessment of preadmission paper work to ensure Residents meet qualifying medical necessity determination
Attend meetings every weekday morning
Qualifications:
RN or good standing with the state licensure board
2 years of long term care experience
Possess excellent assessment and computer skills and use those skills to accurately code the MDS per RAI guidelines
Experienced with State Case Mix management
Good communication skills and the ability to develop a rapport with coworkers, residents, and families
