Admissions-Registered Nurse Job Opening in Stuart, Florida - The Admissions RN conducts the initial admission process for patients referred for hospice services. The Admission nurse initiates the plan of care based on identified immediate needs, and communicates with the IDT.
Utilizing the nursing process, critical thinking skills, collects comprehensive data relating to the hospice diagnosis while conducting a nursing assessment based on hospice protocols and guidelines. Identifies the physical, social and emotional needs of the patient and family. Analyzes the assessment data in determining hospice appropriate diagnosis. Complies with state laws and regulations with respect to the hospice protocol and CMS guidelines for reimbursement. Prepares and maintains accurate medical records, charts and documents to support the diagnosis.
Position to include case management as well as admissions.
Maintains a sense of compassion at all times and is skilled in overcoming objections to hospice services.
Consults and collaborates with the Chief Medical Officer and other physicians.
Performs other duties as directed by the Director of Admissions.
