About the Position:
The Outreach Specialist provides culturally relevant diabetes related education to people at risk of, or who are living with diabetes in order to improve their quality of life. The incumbent provides education, motivation, and support services to individuals referred to the program, both in the home, group, and community setting. Duties include assessing the client?s needs for health and wellness services; conducting basic screening, delivering basic individual nutrition education and lifestyle balance group classes and facilitating diabetes self-management support groups.
Responsibilities & Requirements:
Participates in designing, developing and implementing nutrition and health education and health promotion activities to individuals, and groups across the lifespan in both community and work site settings.
Coordinates with a range of community partners to offer diabetes and wellness screening, prevention education and programs on diabetes self- management best practice.
Provides individualized assessment, goal setting, and offers education, guidance, and teaching.
Diabetes risk factors and prevention strategies, including holistic care;
Special Requirements:
CPR and First Aid Certification are required or must be able to obtain with ninety days of hire. Failure to maintain a current CPR and First Aid Certification may result in termination;
