RN Case Manager - Home Health Job Opening in Wilmington, North carolina - Indeed Hire and AssistedCare are partnering to find a Full-Time Registered Nurse (RN) Case Manafer in the Wilmington, NC area. The ideal candidate hired for the RN Case manager role will provide customized patient care and deliver successful health care outcomes in the private home setting.
Qualifications:
Strong assessment and documentation skills to enable our patients to stay within the comfort of their own homes.
Skilled nursing backgrounds, so clinical experience with PICC lines, dressing changes, wound care and diabetic teaching highly preferred
Develop start of care plans, provide direct patient care during routine visits, and collaborate with clinicians from other disciplines for overall patient wellness.
Working knowledge of OASIS documentation highly preferred

Minimum Requirements:
Valid drivers license
Current North Carolina RN license.
Minimum of 1 year clinical experience in a medical/surgical or Home Health setting
Strong organizational and interpersonal skills.
Ability to see between 5-8 patients a day
Additional information:
Schedule will be Monday - Friday 8am-5pm
This position will require in home visits to patients
Positions also available in Snow Hill, NC and Elm City, NC
