(HPI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HPI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages.
Our full time ACT Advocate, BA - SMI Adults position is located at Adult Outpatient Services in Westland, MI.
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
The Severely Mentally Ill (SMI) Adult ACT Advocate BA shall have, at mini­mum, a Bachelor?s Degree from an accredited college or university, specializing in social sciences. He/she shall obtain and/or maintain current, appropriate issued by the State of Michigan. In addition to documented educational training, the ACT Advocate BA shall have provided direct treatment services to adults in a behavioral health inpatient or outpatient setting for not less than 12 months, of which at least 3 months was spent working with adults with co-occurring disorders, within the past 3 years. In addition to direct work experience, the ACT Advocate BA shall minimally demonstrate (1) basic knowledge of diagnostics, psychopharmacology, and supportive treatment approaches as applied to a severely mentally ill (SMI) adult population; and (2) knowledge of the identifi­cation and treatment of co-occurring mental health and substance abuse disorders. Must have or obtain a Michigan Chauffer's license.

ESSENTIAL JOB RESPONSIBILITIES
Technical:
Intake
Identify issues that may exist regarding safety (personal and environmental), natural supports, and community involvement.
Identify referrals needed for comprehensive treatment, including medical, nutrition, substance abuse, STD/HIV risk, education, and vocational training.
Obtain releases of information for coordination of care related to case management services.
Educate patient and, if applicable, patient guardian with respect to admission infor­mation packet, including but not limited to, Recipient Rights process.
Complete Primary Care Physician Coordination Form.
Demonstrate knowledge of patient payor source during intake process.
Submit Your Choice application, as required, on the day of the initial interview.
Assessment, as required, within seven days of initial interview.
Complete Urine Drug Screen, as indicated.
Schedule appointment for psychiatric evaluation within appropriate timeframe relative to medication supplies and/or acuity of symptoms.
Treatment Planning and Progress Reviews
Complete individualized Treatment Plan for each patient by the fourth treatment session or within 30-days of referral for case management services, whichever is first.
Treatment Plan goals and objectives demonstrate that the plan for treatment was developed with the patient.
Utilize the patient?s Person Centered Questionnaire in the development of the treat­ment plan and support person participation.
Treatment Plan problem areas, goals, and objectives address the patient?s DSM five-axis diagnosis and areas identified on the bio-psychosocial assessment, including but not limited to, personal and environmental safety, relapse or decompensation potential, child and family strengths, and individual needs.
Demonstrate use of natural supports in Treatment Plan.
Treatment Plan goals and objectives represent utilization of community resources and natural supports.
Utilize HPI corporate, continuous Treatment Plan process when receiving or facilitating an internal patient transfer.
Complete Treatment Plan Reviews (TPR) minimally after the first 90 days of treatment and then 180 days thereafter.
Update Treatment Plan via TPR following changes in clinical status, including but not limited to, change in diagnosis, hospitalization, utilization of crisis services.
Complete a Relapse and At-Risk Prevention Plan for all patients on caseload within 30 days of admission.
Inform patients of Medicaid Grievance local dispute resolution process at the time of each Treatment Plan/Treatment Plan Review.
Discharge Process
Document termination of services on the Treatment Plan and on a Progress Note when patient continues to receive other clinical services from HPI.
Document on Treatment Plan criteria and anticipated date for planned discharge.
Update changed discharge criteria on the TPR.
Discharge patients with no contact for 45 days.
Provide written notification to Medicaid recipients of intent to terminate case management services.
Complete Discharge Summary within 21 days of discharge date.
Complete Patient Discharge Demographics form within 21 days of discharge.
Complete a Continuing Care Plan for each patient discharge at the last session for planned discharges and within seven days for unplanned discharges.
Ongoing
Provide individual supportive, solution-focused interventions, not less than twice every 30 days, to all patients on caseload.
Coordinate appointments with psychiatrist and other HPI service providers, as applicable.
Provide linking and coordination of community-based services.
Assist patients and/or adjunctive service providers with various case management tasks, including but not limited to, medication compliance, referrals, and linkage to resources.
Ensure that relevant clinical forms are copied and placed in the records of adjunctive service providers, as applicable.
Provide assistance with patients? Activities of Daily Living as needed.
Transport patients as needed to community-based supportive, adjunctive services.
Follow 24-hour crisis on-call schedule.
Participate in daily team meetings.
Meet with patient in hospital within two (2) working days of notification of hospitalization.
Meet and discuss aftercare with inpatient social worker at the time of the initial face-to-face contact with the patient.
Contact CareLink Clinical Care Manager regarding aftercare service coordination for non-Medicare, HPI/CareLink patients on caseload.
Complete Clinical Care Manager Review Form for inpatient admissions at the time of the initial face-to-face contact with the patient and submit to the appropriate CareLink staff member.
Ensure a minimum of three contacts with post-hospitalized patients, for both community and state hospital discharges, within ten days of hospital discharge.
Meet with patients discharged from state hospitals no less than one time per week for the first two months after discharge, and no less than one time per month thereafter for the first six months, at minimum, after discharge.
Legibly document using language meeting professional standards, abbreviations per HPI approved listings, and error correction processes per policy.
Type external documentation on HPI issued letterhead or clinical document.
Initiate contact, in person, by phone, or in writing, to non-compliant patients within 24-hours of a missed appointment.
Assist with problem solving and behavior management concerns in regards to patient maladaptive behaviors or decompensation.
Complete a progress note for all treatment services that include clinical presentation, a summary of clinical content of the session as it relates to the patient?s identified problems, medication compliance, and progress toward the goals/ objectives written on the patient?s treatment plan.
Each day worked, complete and submit (within 24 hours) a Daily Service Record corresponding to patient services documented in each patient?s record.
Report medication issues, including non-compliance and adverse side effects, to clinic psychiatrist and/or clinic supervisor immediately.
Consult with clinic psychiatrist within five days of the identification on a non-emergent medical condition.
Monitor substance use via Urine Drug Screens, as indicated.
Inform patient and support persons(s) regarding the process involved in acquiring Clozaril medication.
Monitor patient progress including medication compliance and compliance with scheduled blood draws.
Assist patients with application process for Patient Assistance Programs, track and monitor application process.
Complete required annual documentation updates and/or referrals, per HPI policy.
Complete annual BAPTP Update within 13 months of admission.
Provide clinical supervisor with outlines and written handouts for all psycho-educational didactic group sessions prior to implementation.
Interpersonal:
Establish effective therapeutic bonds with patients assigned to caseload.
Maintain therapeutic relationships within boundaries established per licensing guide­lines and HPI ethical pledge.
Maintain collegial, cooperative relationships with peer therapists and other staff of HPI.
Report all Recipient Rights complaints and knowledge of potential com­plaints to clinical supervisor within 24 hours of said knowledge.
Contribute pertinent clinical information at regular case conference meetings.
Annually attend a minimum of 10 monthly staff meetings.
Follow directives provided in staff meetings.
Respond to written requests from clinical supervisor or HPI Administration regarding clinical and/or operational information within indicated time frames. Must utilize indicated formats, if applicable.
Critical Thinking:
Provide services at frequency and intensity consistent with individual patient needs, but no less than two contacts per month.
Provide clinic supervisor, psychiatrist, and applicable adjunctive service providers with timely, pertinent information regarding changes in patient(s) condition(s).
Provide clinic supervisor with timely, pertinent information regarding a patient?s state of mind that has potential impact on the safety of the clinic environment.
Advise clinic supervisor of caseload difficulties in advance of scheduled time off.
Consult with clinic supervisor and primary therapist in the event a patient presents at risk of harming self or others, either intentionally or otherwise.
Be knowledgeable of potential side effects or adverse reactions of psychotropic medications.
Maintain continuity in approach to treatment in all areas (Medication Assistance, Medicaid Application, Case Management Assessment, Treatment Plan goals and objectives, progress notes, and community referrals).
Productivity:
Adult Population: Maintain a minimum of 52.5% direct services.
Physical Requirements:
Must have adequate visual acuity to interact with the Electronic Health Record System and accurately input data.
Regular and reliable attendance and punctuality.
