Mental Health Aide/ Case Manager Job Opening in Nottingham, Maryland - To provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of consumer needs. The overall goal of the position is to enhance the quality of consumer satisfaction, to promote recovery and continuity of care through the integrating of services and functions of community, utilization review and management and discharge planning. Has accountability for the care, coordination and discharge planning of all consumers. This position is accountable to the Program Director.
QUALIFICATIONS
Associate degree in healthcare related field preferred. High school diploma and 40 Hours of PRP training required (may be provided by agency). Minimum of one (1) year experience in the field is preferred. Clean background check and a valid driver's license is a must.
SUMMARY OF MAJOR AND ESSENTIAL FUNCTIONS
Provides agency with case review and discharge planning to assure that the consumer progresses with PRP goals and is discharged to the least restrictive environment. Coordinate the integration of the social service function into consumer care. Coordinate the activities concerned with case management and discharge planning. Adhere to consumer goals, objectives, standards of performance, and policies and procedures. Ensure compliance with quality consumer care and regulatory compliance.
DUTIES AND RESONSIBILITIES
Coordinate the integration of social services/case management functions into the consumer care, discharge, and home planning processes with other community supports, external service organizations, agencies and healthcare facilities. Conduct concurrent review using specific indicators and criteria as approved by administrative staff. Act as consumer advocate. Promote effective and efficient utilization of clinical resources. Mobilizes resources and interviews, as needed, to achieve expected goal to assist in achieving desired clinical outcomes within the desired time frame. Conducts review for appropriate utilization of services from admission through discharge. Evaluate consumer satisfaction and quality of care provided. Communicates with consumer resources at regular intervals throughout consumer recovery, develops an effective working relationship. Complete expanded assessment of consumer and family needs at time of admission. Complete psycho-social assessment. Assess consumer?s progress through expected course of recovery. Refers cases where consumers would benefit from other services. Case manager enhances a collaborative relationship to maximize the consumer?s ability to make informed decisions. Collaborate with clinical staff in the development and execution of the IRP, and achievement of goals. Directs and participates in the development and implementation of consumer care policies and protocols in order to provide advice and guidance in handling special cases or needs. Coordinates the provision of social services for consumers .
