POSITION SUMMARY Under the general direction of a Compadres Clinical Program Manager, and requiring independent judgment in the application of established procedures, collaboratively works with the Parent Partner as a Care Coordinator Pair. The facilitator, accompanied by the Parent Partner, is responsible for rapid engagement of the family in wraparound services, develop and then implement crisis stabilization services and plans of care. The Facilitator is also responsible for interfacing with the Provider Network as well as the Action Counsel and other formal oversight bodies. Performance of the typical responsibilities and duties requires the ability to perform the job duties, explain to/instruct others, persuade others, involving job-related matters of moderate complexity. TYPICAL RESPONSIBILITIES AND DUTIES (*: The non-essential functions) 1. Provides a high level of Wraparound services including rapid response, as part of a Care Coordination Pair to approximately 14 children and families including but not limited to 24/7 crisis support and stabilization services. 2. Collaboratively works with at Parent Partner to demonstrate effective parent/professional partnerships to further engagement and transition through the phases of the Wraparound process. 3. Develops and utilizes Wraparound services and best practices to formulate a creative strengths based integrated Family Plan of Care with input from all team members. 4. Provides respectful, culturally relevant services to children and their families in community-based settings, and in-home services. 5. Coordinates child and family teams to work with the youth and their supports to identify their strengths, needs and local resources to meet those needs. 6. Demonstrates team leadership and advocacy in empowering family and support people in attaining permanency, safety, well-being and their self-determined quality of life. 7. Assists the child and family in planning for graduation from Wraparound service and referral for aftercare services. 8. Works collaboratively with other agency staff and with individuals and agencies in the community to ensure relevance, accessibility, and quality of service provided to children and their families. Works to evaluate program performance, collaboratively problem solve, and explore all possible solutions. 9. Meets expectations and deadlines to submit proper electronic and paper documentation to the Wraparound Administrative Service Agency and DSES designee. This includes report writing and accurate data collection as necessary to assess progress and outcome. 10. Assists in the development and refinement of existing documentation and data collection systems. Documents charts according to the State and County requirements. 11. Maintains professional standards regarding client rights. 12. Demonstrates an understanding of confidentiality policies, mandated child abuse reporting and duty to warn procedures. Utilizes clinical supervision in any case where these issues are in question. 13. Prepares family plan presentations for DSES, Department of Behavioral Health, JPD, and other community partners as appropriate 14. Utilizes supervision, consultation and training as necessary to continue to provide high quality services to children and their families as effectively and efficiently as possible. 15. Prioritizes utilization of community linkage resources to best meet the needs of the children and families served over the use of fee based resources. 16. Adheres to Wraparound principles with regards to financial support and the use of program Flex Funds and utilization of provider network resources. MINIMUM QUALIFICATIONS 1. Education, Training, and Experience. a) Current BSW or masters level student or equivalent. b) One year of experience working with high-needs children and families. Experience working in a children�s social services setting emphasizing a family systems perspective is also preferred. c) Skills in crisis intervention and stabilization, suicide assessment, conflict resolution, and interpersonal communication are preferred. d) Bilingual/ Bicultural (Spanish/English) required) Experience working with schools, Juvenile Probation, Department of Social and Educational Services, Department of Behavioral Health, and other larger county systems. 2. License(s). a) California driver�s license (and good DMV report), ability to be insured by agency insurance carrier. 3. Knowledge and Skills: a) Leadership and organizational skills. b) Program development skills and Wraparound knowledge. c) Strong ability to work as part of a team. d) Ability to complete Assessments and develop and implement individualized Family Support Plans and Treatment Plans. e) Excellent time management skills. f) Foster care system knowledge. g) Knowledge of and ability to access other community resources. h) Knowledge of documentation procedures for both Wraparound and ETO. i) Ability to work with a balance of independence and creative use of supervision from the Clinical Program Manager. j) Ability to think creatively and intervene/respond in crisis situations. 4. Special Requirements Must be able to meet and receive a criminal records clearance as required by Title XXII licensing regulations. 5. Physical and Mental Requirements. The following are required in day-to-day performance of the duties of this position: a) Sitting, walking, standing, grasping (simple), and visual/auditory acuity are constantly required (over 2/3 of the workday). b) Bending, squatting, kneeling, twisting, lifting, reaching and pushing/pulling are required occasionally (under 1/3 of the workday). c) Climbing and crawling are generally not required. monterey ca california
