Insight Memory Care Center, is a 501(c) 3 nonprofit whose mission is to provide specialized care, support and education for individuals with Alzheimer's disease and other memory impairments, their families, caregivers, and the community through our adult day health and resource center. Serving Northern Virginia since 1984, IMCC offers a spectrum of holistic care, education and support, with a vision of a community where those affected by memory impairments can achieve the highest quality of life. IMCC also provides a safe and therapeutic environment for people in all stages of dementia. A vital part of IMCC is providing a well-rounded therapeutic recreation program that meets the physical, cognitive and social needs of each participant.

IMCC is seeking a Social Work Coordinator who is part of a team that interfaces with the participants and their caregivers, local geriatric healthcare professionals, and the community at large to promote the work of the Center. The Social Work Coordinator is responsible for facilitating the SHARE program, support groups, family consultations, select early stage programming, community outreach, and other duties that further the mission of the Center. This position reports directly to the Education and Outreach Coordinator.
The Social Work Coordinator will assist in implementing the SHARE program. SHARE is a research-tested program developed by dementia experts, provides guided discussion sessions where families receive individualized support and information focused around building a SHARE Plan, which includes both partners? preferences for who will help support them if and when assistance is needed in the future. Rather than waiting for a crisis to occur, the SHARE program encourages families to create a plan for future care, and provides the guidance and resources to do so.

Key Responsibilities:
Below are the main duties and responsibilities of the Social Work Coordinator:
Implement and facilitate SHARE Program at IMCC.
Meet with prospective participants for SHARE Program, assessing for appropriateness to program.
Maintain communication with family caregivers concerning the well-being of participants.
Serve as the contact person for family concerns about the program.
Develop and maintain knowledge base of community resources and care providers for those with dementia.
Participate in advertising and promotion of the program.
Seek out, arrange for, and conduct educational presentations on the SHARE program to physicians, to places of worship, community groups, professional organizations, community forums, health fairs, meetings etc.
Be available to speak about the overall IMCC program to those same groups, bringing the message of IMCC?s services to a broad audience.
Participate in various collaborative efforts with other professionals in long term care to develop and conduct education programs for the general public.
Conduct monthly support group for caregivers as assigned
Talk with prospective members, assessing appropriateness to groups.
Provide individual and family care consultation for the general community, as well as participant families, as requested.
Assist as needed at our Mind and Body Workshop and our Memory Café.
Conduct monthly support group meetings for Reconnections members.
Education
Graduation from college with a Bachelor?s Degree in Social Work, Gerontology, Psychology, Health Services, or related field. Master?s degree preferred.
Experience
Experience in public speaking, support group facilitation, extensive experience in dementia and various other types of memory impairments (4 years minimum). Also experience in planning and executing educational classes for family caregivers, professional caregivers, and providing 1:1 consultations. Experience in long-term care settings or Adult Day Health Care services. Knowledge of Northern Virginia dementia resources.
