POSITION SUMMARY : Practitioner is a Medical Doctor who is skilled in evaluation, diagnosis, treatment, and provision of consultation for patients presenting with mental, behavioral, addictive or emotional disorders, e.g., psychoses, depression, anxiety disorders, substance abuse disorders, developmental disabilities, sexual dysfunctions, and adjustment disorders. Includes general medical management of patients concurrent with psychiatry privileges. Assess, stabilize, and determine disposition of patients with emergent conditions consistent with Clinical Staff policy regarding emergency and consultative call services. The incumbent collaborates with other health care providers, social workers, and other professionals to discuss treatment plans and patient progress.

This position requires cooperation with the Agency?s Quality Improvement Program. This includes actively participating in all required trainings, and assisting in creating a culture of quality improvement at the Agency. Cooperation with the quality program will be an element considered as part of all performance evaluations.
This position requires cooperation with the Agency?s Compliance Program. This includes immediately reporting any concerns, actively participating in all required trainings, following HIPAA regulations, and assisting in creating a culture of compliance at the Agency. This also includes compliance with the written standards of the Compliance Program, and all applicable federal and state laws. Cooperation with the compliance program will be an element considered as part of all performance evaluations.
This position requires cooperation with CrescentCare Compliance policies and procedures that support its status as a ?deemed? entity under the Federal Tort Claims Act (FTCA), specifically related to covered services, health center patients and FQHC-approved service delivery sites. CrescentCare provides guidance and training to assure that all practitioners are aware of services covered under its approved scope of services, the sites at which services may be provided, and the factors that define a CrescentCare patient. FTCA coverage requires that practitioners provide care within their defined scope of practice and as privileged by CrescentCare.
Failure to comply with the Agency?s Compliance Program, which may include the failure to report any conduct or event that potentially violates any laws or the Agency?s written compliance standards, will be met by the enforcement of disciplinary action, up to and including possible termination.
ESSENTIAL PERFORMANCE STANDARDS
* Obtains and documents a health history;
Performs and documents complete, system-focused, or symptom-specific examination;
Assesses the need for and performs additional screening and diagnostic testing, based on initial assessment findings;
Prioritizes accurate data collection;
Orders, executes, and manages appropriate clinical tests; analyzes and interprets diagnostic findings; formulates differential diagnoses by priority;
Conducts patient therapy session for assigned patient population;
Documents progress notes, plan of care, evaluation and discharge summary appropriately in the EHR for medical record keeping, compliance, billing and reporting purposes;
Administers or prescribes appropriate pharmacologic and non-pharmacologic treatment modalities;
Utilizes evidence-based, approved practice protocols in planning and implementing care;
Initiates appropriate referrals and consultations;
Provides specialty specific consultation services upon request and within specialty scope of practice;
Assists with volatile patients when needed;
Facilitates the patient?s transition between and within health care settings, such as referring, transferring, and discharging patients;
Maintains clinical competency, as demonstrated in providing and directing patient care;
Maintains compliance with FTCA policies, LSBME, OSHA and other regulatory agencies? statutes and standards including CrescentCare Standards of Care and CrescentCare Clinical Staff Guidelines;
Complies with all Agency and regulatory safety standards and infection control;
Assists in educational programs for staff and participates in team meetings;
Obtains proper informed consent as a prerequisite to any procedure requiring informed consent. This information must include the signature of the responsible practitioner.
Provides education to patient and patient?s personal support system;
Communicates with medical staff concerning patient care issues and functions as a part of a Care Team, using evidence-based practices;
Promotes teamwork, good interpersonal relations and communication among staff at the healthcare setting;
Follows CrescentCare policies and procedures related to patient care;
Operates within the scope of practice as outlined by the privileges granted and this job description;
Participates as required in Peer Review process;
Designs individualized care plans, using a variety of treatments based on patient history;
Analyzes and evaluates patient data and tests, or examines findings to diagnose nature and extent of brain injury or mental disorder;
Prescribes, directs, and administers psychotherapeutic treatments or medications to treat mental, emotional, or behavioral disorders. ssumes additional responsibilities as necessary;
Works closely with the Behavioral Health Teams
Participate in Health Fairs, employee and volunteer health programs, immunization campaigns, and home visits as appropriate;
Other duties as assigned.
POSITION REQUIREMENTS
Education, Work Experience, Knowledge & Skills
Graduate from medical school approved by the Liaison Committee on Medical Education of the American Medical Association OR the American Osteopathic Association OR possess certification by the educational Council for Foreign Medical Graduates (ECFMG).
Successful completion of an ACGME or AOA-accredited in Psychiatry, or foreign equivalent training, with a current specialty certification in psychiatry by the American Board of Psychiatry and Neurology (ABPN) or the American Osteopathic Board of Neurology and Psychiatry (AOBNP) or is in active participation in the examination process.
Has admitting privileges with a hospital directly or a collaborative agreement with a psychiatrist or neurologist who has admitting privileges.
Periodic hospital call or hospital emergency room coverage, if it is required by the hospital as a condition for obtaining hospital admitting privileges.
Provision of individual emergency treatment when, (if the provider is already providing or undertaking covered services), such is a condition of employment
BLS / ACLS certifications as necessary
Ability to work flexible hours
Holds current Louisiana Medical Licenses without restrictions
Must have pertinent clinical experience within the past 2 years sufficient to demonstrate current clinical Psychiatry proficiency. Child psychiatry experience desired.
Necessary technical skills to use Agency?s electronic health record, incident reporting system, email, and other information systems.
WORK CONDITIONS
Work with a diverse population as served by a Federally Qualified Health Center
Able to use a computer to chart patient information in the Electronic Health Record.
Physical ability to participate in training sessions, presentations, and meetings.
Must be willing to work a flexible schedule and weekend hours.
May be exposed to infectious diseases
Some travel may be required.
Position requires light physical effort, frequent prolonged standing/walking, may occasionally require lifting of supplies/equipment, occasional reaching, stooping, bending, kneeling, and crouching.
