Service Coordinator Job Opening in Orangeburg, South carolina - New Vision is an inpatient medical stabilization service for adults with drug, alcohol and related health issues. The service accepts appropriate adults, who are currently using or are experiencing acute withdrawal symptoms from certain drugs. New Vision has a fully qualified treatment team including an attending physician and nursing personnel who utilize a multidisciplinary team approach in the stabilization of the patient.
Position Purpose : The Service Coordinator is responsible for the management of the New Vision office, which includes the oversight of the discharge planning process. The Service Coordinator is also responsible for planning outreach activities and initiating contacts in the community to enhance the visibility of the service and hospital. The Service Coordinator works collaboratively with hospital administrators, the nursing leadership, and the Director of Clinical Services to coordinate a continuum of care for the addicted patient.
Essential Functions for Community Outreach
Functions as a department head within the hospital in order to coordinate the operation of New Vision service effectively.
Provide on-going training to New Vision and hospital staff regarding criteria for admission as required by Medicaid, Medicare and insurance plans.
Meet routinely with assigned hospital contacts to ensure ongoing communication between the hospital and SpecialCare Hospital Management,
Maintaining and expanding knowledge and understanding of the field of chemical dependency and social service,
Functions as a liaison between the service and the community making a minimum of 12 face to face contacts per week
Qualifications:
Education/ Minimum of a Bachelor?s degree in related fields such as Human Services, Credentials: Psychology, Counseling, Substance Abuse, Criminal Justice, or Social Work; Licensed professionals preferred.
Required Knowledge:
A thorough understanding of chemical dependency or behavioral health.
Knowledge of all related computer applications.
Understanding of social services and hospital procedures and policies
Required Experience:
Minimum of at least three (3) years of social service and/or related healthcare experience in a medical field required
Prior experience in the field of substance abuse
Marketing and public relations that involved planned community education
Certification as a chemical dependency counselor preferred
