The RN Care Coordinator's primary job function is to provide, implement, direct, evaluate, delegate to qualified personnel, and coordinate nursing care while helping facilitate patient access and flow within the pod.
Job Specific Competencies
Care Coordination - Patient Centered Care including goal setting and follow-up:
Advocates for patients, assisting with and connecting the patient with services and resources required to maintain health and wellness.
Responsible for assessment of medical and psychosocial needs and planning follow-up education, including nursing advice or scheduling CVCH visits for patients.
Evaluates Chronic Care disease management and Health Care Home indicators to identify at-risk patient groups to improve measureable outcomes.
Assists in scheduling, finding transportation and other resources for patients with complex referral needs.
Coaches patients with identified self-management goals or specific care plans with identified goals using phone calls and/or e-mails to check on progress toward the goals.
Provides education related to the patient?s needs and assures that the team has the educational materials required and shares information appropriately.
