Per Diem Case Manager (Union/Morris County) Job Opening in Denville, New jersey - Roots & Wings provides young adults who age out of the foster care system in New Jersey with safe housing, education, counseling, and life skills in order to empower them toward self-sufficiency.
Roots & Wings is a 501(c)(3) nonprofit organization that was established in 1999. What started as a collaborative, community-based volunteer operation has grown into a well-established organization made up of professional staff, trained mentors and dedicated volunteers. From the beginning, Roots & Wings has been client-centered, providing nurturing support, safe housing, educational support and practical life skills to young adults who have aged out of the child welfare system. Roots & Wings provides a foundation for this high-risk population of disadvantaged young adults on which they can build ? replacing dependency with self-sufficiency.
The Case Manager is responsible for a portfolio of Union and Morris County based client cases at Roots & Wings. This position will have office space in Denville, NJ and field work in Garwood, NJ.
Specific responsibilities include, but are not limited to:
1) Providing nurturing support, practical life skills, and increasing the independence of the clients in the community.
2) Supportive individual contact, transportation, case management, psycho-education, and completing necessary documentation.
3) Adhering to Roots and Wings organizational policies and ethical standards. Observing, maintaining, and complying with all standard ethical practices required by Roots and Wings, as well as, the laws and regulations which govern the overall mental and healthcare industries
4) Acting in a professional manner and carrying out Roots and Wings mission
