Community Outreach Liaison Job Opening in Skokie, Illinois - The Community Outreach Liaison provides professional and community outreach to hospital/healthcare, business, eldercare and other professional and community members who are deemed to have opportunities for referring to or utilizing CJE SeniorLife?s businesses and services for the older adult or themselves. He/she will also provide assistance to various CJE business lines to ensure smooth transition into the appropriate level of care either within or outside CJE network as appropriate. The Community Outreach Liaison will work collaboratively with the CJE Marketing Team and Agency programs to meet and exceed business, marketing and occupancy goals.
Essential Duties and Responsibilities for a Community Outreach Liaison:
Understand and able to clearly articulate the most pertinent details of all CJE programs and services.
Able to define general and specific target audiences for outreach efforts by program as necessary.
Market all CJE programs and services as appropriate for each referral source including inter-agency or internal referrals.
Maintains and/or improves established working relationships with current referral sources for all programs, to include discharge planners, physicians, senior centers, other continuity of care professionals, etc.
Identifies, establishes and cultivates good working relationships with new referral sources for all programs.
Keeps abreast of new business opportunities and provides form external market to Marketing Manager, Sales and Customer Service.
Implements a consistent schedule of follow-up to contact interaction to encourage face-to-face meetings and relationship cultivation in order to meet determined monthly goals.
Enters all outreach activity in ACT! database daily, including but not limited to: face-to-face meetings, phone calls, referrals and admissions.
Create outcome reports in alignment with monthly goals, as directed, to share with Management Team.
Visits hospitalized residents as necessary for Lieberman Center for Health and Rehabilitation or other programs.
Visit new inquiries prior to admission to Lieberman Center for Health and Rehabilitation and other programs to assist with assessments for appropriate placement in said program as needed.
Encourages referrals to Lieberman Center for Health and Rehabilitation from target hospitals to encourage positive working relationships to meet business, marketing and occupancy goals.
Take ownership of and assist with professional and community events including but not limited to: CEU presentations, grand openings, vendor fairs, etc., as directed by the Manager.
Maintains membership with appropriate COC?s and other networking groups.
Work in collaboration with various program managers to develop outreach plan to meet business, marketing and occupancy goals as directed by Manager.
Create and implement weekly, monthly, quarterly and annual marketing plan focused on outreach and business development in collaboration with Manager and program managers as appropriate.
Work in collaboration with Manager in leading sales meetings at various programs as needed.
Prepare competitive analyses as necessary.
Work in collaboration with CJE Marketing team for best practices and updating collateral and other resources as necessary.
Qualifications for a Community Outreach Liaison:
Bachelor?s degree preferred with degree in gerontology, marketing or social work
Five or more years of working experience in B2B, B2C, marketing within the healthcare or geriatric marketplace. Skilled nursing knowledge a plus.
Good verbal, written, presentation and communication skills.
Solid ability to interact/build relationships with various internal and external customers.
Strong organizational skills with the ability to work independently.
Proficient in Word, Excel, Outlook, knowledge of ACT! or similar contact database.
Competency and proficiency with computer and smartphone skills.
Flexibility with schedule as needed.
Self-motivated/team player, accountable.
Must have own reliable transportation with regular availability to travel throughout assigned region.
