Job Description:
The External Case Manager will screen patients for medical appropriateness and refer cases to achieve optimal clinical and financial outcomes, utilization and services. Builds and maintains relationships with other health care providers promoting the facility to physicians, patients, family members and hospital case managers, working with the hospital discharge team to ensure a smooth transition to continuing care.
Experience and existing relationships in hospitals preferred.
