A RN Case Manager is a pivotal member of the interdisciplinary team and is the point person in identifying the physical, psychological, social, and spiritual needs of the patient and family. The RN is responsible for initiating the appropriate intervention and support for the patient and family upon admission to Hospice and provides a continuously appropriate, comprehensive, and responsive plan of care.
Minimum Qualifications:
Licensed as a Registered Nurse in Oregon
BLS Certification
Current State drivers' license, and reliable transportation with minimum state required insurance
Experience in patient education, planning, and management desired
Exceptional communication and customer service skills
Proficiency with technology, including computers and smart phones
Exhibit regular and prompt attendance
Exhibit a professional image and maintain a positive and respectful attitude

This is a salary, exempt position with additional compensation for Call coverage. Accrual begins on the date-of-hire, and benefits are effective the first of the month following 60 days of employment.
