Job Type: part-time
The Visit Nurse (RN) or (LPN) must be client focused, have a passion for patient outcomes, and have the skill of motivating and influencing employees. The Visit Nurse works with the supervision of the Clinical Supervisor to implement care plans for their clients. The Visit Nurse ensures that services are delivered according to licensing guidelines, professional medical standards and agency policies and procedures while following physicians orders.
Professional Responsibilities
Ensures that aides are properly oriented and trained to meet the client's needs in accordance with agency policies and procedures
Instructs clients, family members and primary care givers as needed to insure the maintenance of or acquisition of optimal functioning level for each client
Ensures effective coordination of client home care services through the timely completion of required documentation as well as the timely transfer of pertinent medical data to the client's physician, therapists or other care givers
Maintains accurate and comprehensive client medical data
Routinely reviews pertinent client medical data to determine the effectiveness of services in reaching maximum rehab potential
Skilled 1:1 nursing with In-home patients
Specific Requirements of the Visit Nurse (RN or LPN)
Current Registered Nurse or Licensed Practical Nurse (RN/LPN) license in good standing
Minimum of one year (RN/LPN) Home Health Care Experience or LTC Experience a plus
Strong assessment and organizational skills
Available for on-call medical questions and/or emergencies from Recover Health Staff
Ability to Multi-task and work under time constraints
Proactive approach to intake management demonstrating leadership skills at all times
Must have a proven, successful leadership background, ability to mentor
