ChartSpan is revolutionizing the world of chronic care management and we want you to be a vital part of our team as a Clinical Support Specialist.
A Clinical Support Specialist coordinates care for patients; providing an essential service which helps providers stay in touch with and meet their patients' healthcare needs in-between office visits. Your role is to support and assist patients in obtaining the resources they need to improve upon their health, happiness, and longevity.
From our call center in downtown Greenville, you will be speaking with and helping patients over the phone, garner a more comprehensive understanding of their needs and concerns, update their medical records, request specialist referrals, process prescription refills, assist with medication adherence, schedule future appointments, conduct assessments, and help patients manage their care goals. You will be the heartbeat of each patient?s preventative care and will have the opportunity to help hundreds of patients while making an enormous impact on the health of chronically ill patients.
What does it take to be successful at ChartSpan?
Be a patient advocate
Demonstrate sensitivity and empathy to chronically ill patients
Protect patient confidentiality
Possess a current license as a Certified Nursing Assistant (CNA), Certified Medical Assistant (AAMA), Registered Medical Assistant (RMA) or Licensed Practical Nurse (LPN)
Able to work a schedule of 8:00am - 5:00pm, Monday through Friday
First-rate triage skills
Demonstrate excellent verbal skills
Wow and delight the patient with excellent service and care - as if they were a member of your own family
Show us your awesome typing and data entry skills
Call center experience is a plus!
