Description: As a Registered Nurse (RN) Case Manager you will be responsible for acute assessments, supervisory skilled nursing visits, rounding with MD, care plans, teaching and training Licensed Practical Nurses and Universal Workers, Phlebotomy training, employee evaluations and more. To qualify for the Registered Nurse Case Manager position, you must have effective communication skills and be able to work with individuals of all ages.
Additional requirements include:
Current Minnesota Nursing License in good standing with the Board of Nursing
Must have at least three years of experience in geriatric, long term care or similar nursing experience is preferred
Valid Minnesota Driver?s License
Experience in LTC and/or HHC
Good organizational skills and ability to multi-task throughout the day
Must be willing to travel between three health care facilities in Central Minnesota
Additional Information:
The RN Case Manager position is Full-Time, on-call every other weekend.
