Quality Improvement Specialist Job Opening in Minneapolis, Minnesota - Job Summary
The Quality Improvement Specialist supports the priorities and activities within MNACHC?s Quality Improvement program. This position will work to improve the quality of care provided at Minnesota?s health centers by supporting their utilization of health information technology, application of quality improvement science, use of data analytics for improving population health, and optimization of innovative care models.

Key Responsibilities
Assist in the coordination and facilitation of peer learning roundtables for health center clinical and quality improvement staff.
Arrange trainings/webinars as needed to support health center quality improvement efforts.
Assist in providing technical assistance and disseminating resources to health centers in support of their quality improvement work (EHR optimization, PCMH/HCH, quality payment programs, data analytics, population health, and other clinically focused areas).
Coordinate data collection, develop reports, and provide analysis of health center quality, operational, and financial measurement data.
Identify and share best practices and lessons learned to achieve improvement in healthcare delivery systems, processes, and outcomes.
Prioritize, anticipate, and asses the QI needs of health centers.
Assist in providing technical assistance in meeting Bureau of Primary Health Care (BPHC) clinical program requirements related to QA/QI plans, Federal Tort Claims Act (FTCA), risk management, 340B Pharmacy Program, and Uniform Data System (UDS) clinical measures.
Assist in providing technical assistance to health centers related to emergency preparedness requirements.
Other duties as assigned.
Qualifications
Bachelor?s degree in health information management, health care management, nursing, public health or other related health care field and 2+ years? experience with implementing quality improvement initiatives in clinical settings.
Experience with electronic health records and quality
Experience with health data analytics and use of data to drive performance.
Excellent writing, presentation and analytical skills.
Strong interpersonal skills and ability to work collaboratively in a team-based environment.
Demonstrated ability to think and act strategically and proactively.
Ability to travel to FQHCs within Minnesota and to occasional state, regional and national meetings.
Proficient in commonly used software products (e.g. Excel).
Preferred Qualifications
Experience working in an FQHC or other safety net health care setting.
Experience with team based care models (Patient-Centered Medical Home and/or Health Care Home).
Understanding of the Social Determinants of Health and the impact of health disparities on racial and ethnic populations.

Salary: $40,000.00 to $45,000.00 /year
