RN Case Manager, memory care residence Job Opening in minneapolis, Minnesota - RN Case Manager, memory care residence Join this terrific team of health care professionals who provide services to those living at The Alton, a memory care residence located in St. Paul?s Highland Park area. This location is easily accessible via 35E, 494, 5 and Crosstown 62 ? you can get here from just about anywhere! The members of our RN Case Manager team provide direct care to residents and serve as leaders for the LPNs and Home Health Aides. The work schedule is 32 to 40 hours per week (four to five eight-hour days per week) some time during Monday through Friday daytime hours, with on-call responsibilities every three to four weekends. In addition to providing services to those living at The Alton, our RNs occasionally provide services to those living at The Wellington, a neighboring assisted living residence. We offer a competitive pay rate that is dependent upon qualifications.
