Direct Care Professional Job Opening in Kansas City, Missouri - Preferred Family Healthcare is seeking a TBI Direct Care Worker. This positions is responsible for providing assistance to individuals with traumatic brain injury regarding activities related to daily living, maintenance of household and community integration consistent with and according to the individual?s plan and the authorized services. He/she will also be responsible for daily, weekly and or monthly documentation. Essential duties and responsibilities include the following. Assists persons served in developing and achieving goals defined in their individual plans. Compiles and reviews pertinent referral information to include medical and psychological information. Uses this information to understand the nature and residuals of the brain injury thereby developing and implementing pertinent compensatory strategies necessary to support the person served in meeting the goals and objectives of their plan. Will periodically review plans of persons served to determine any necessary adjustments to the plan. Will monitor contract compliance and ensure that utilized hours in supporting persons served do not exceed authorized hours. Will teach the skills each person needs support with according to the individualized plan. Will complete reports and data collection summaries as required. Will transport persons served as necessary to carry out goals and objectives of the individualized plan. Will regularly communicate the plans, services and progress of persons served to the client's Service Coordinator, Manager and Regional Director. Will participate in scheduled meetings as required. Will serve as advocate or liaison between persons served, referral sources, business community and other social service agencies. Must have high school diploma or equivalent. Must have driver's license, reliable transportation and proof of insurance. Minimum of one to two years experience in human services field preferred, particularly serving individuals with traumatic brain injury. Must successfully complete specified training in the area of traumatic brain injury as specified by contractual guidelines. TBI Direct Care Worker will support individuals throughout the KC MO area in their local community.
