Registered Nurse/Case Manager Job Opening in East Providence, Rhode island - Care Resource is a locally owned and managed Medicare Certified Home Health Agency and Medical Supply company providing state-of-the-art technology and advanced clinical programs for patients throughout Southern New England.
We are seeking Full Time Registered Nurses to join our team at our East Providence, RI office.
POSITION SUMMARY :
A Registered Nurse who is responsible for planning, coordinating, providing and documenting care for a geographically assigned caseload of patients from day of admission through day of discharge. Utilizing physicians? orders and professional skills, the Home Health Agency Case Manager RN develops and implements a home health plan that meets each patient?s specific needs and is in compliance with accreditation standards, Medicare Oasis guidelines, federal and state regulations, reimbursement guidelines as well as Agency policies and procedures.
Obtains all necessary physician orders and collaborates with physician as client's needs dictate.
Re-evaluates the need for continued care on an ongoing basis; initiates recertification as necessary.
Provides nursing care to all ages of clients from neonate to the geriatric client.
Knowledgeable in the use of medical equipment. (Wound Vac, Pleura-Vac, etc)
Provides education to client and/or caregiver on treatment and ongoing care.
Follows up with obtaining lab results and notifying physician of any significant results.
Maintains aseptic practices and infection control procedures.
Initiates case conferences to discuss multidisciplinary team responsibilities, client progress, plans for continued care, new problems, etc.
Participates in agency?s mandated activities, including in-services, UR/PI committee meetings and staff meetings.
Meets established guidelines for documentation.
Ability to handle stress and meet deadlines
Possesses the ability to deal tactfully with personnel, referral sources, patients, family members, visitors, government agencies and the general public
Exhibits efficient problem solving skills. Is organized and works well independently as well as in groups.
QUALIFICATIONS : Minimum Requirements
Licensed Clinician in the state of practice with 2-3 years of experience in the Home Care setting.
Current BCLS certification.
Current health examination and TB testing
Valid and current driver's license and evidence of automobile insurance
Ability to travel to multiple locations, cover specified geographical services area and attend required meetings Preferred Qualifications:
Associates, Bachelors or Advanced degree in a health care field
Current certification as CRNI/Wound Care
Home Health Care experience preferred
