Job Description :
The purpose of the Home Visiting Team Lead position is to ensure that Indiana families have access to a range of high quality, effective and coordinated evidence-based home visiting services. The Home Visiting Team Lead is responsible for providing oversight, supervision and direction to the Home Visiting Service Coordinator and the MIECHV Grant Coordinator as well as coordinating ISDH MCH home visiting services with other evidence-based and promising practice home visiting programs available in Indiana.
The Home Visiting Service Coordinator is responsible for coordinating implementation, on-going monitoring and support of Nurse Family Partnership (NFP) and other home and community-based providers for ISDH MCH. The Home Visiting Service Coordinator is responsible for ensuring that all ISDH funded NFP and other MCH related home and community-based programs are implemented in a manner that meets all guidelines for model fidelity as well as all related Federal and State requirements. In addition, the Home Visiting Service Coordinator will work closely with state and local partners to ensure all ISDH funded programs operate in a coordinated and cooperative manner with other similar programs available in Indiana. The Home Visiting Service Coordinator works under the direct supervision of the Home Visiting Team Lead.
The MIECHV Grant Coordinator for ISDH MCH is responsible for coordinating all aspects of grant level implementation, benchmark reporting and continuous quality improvement of the MIECHV program. The Coordinator is responsible for ensuring that MIECHV is implemented in a manner that meets all Federal and State requirements. In addition, the MIECHV Grant Coordinator for ISDH MCH will work closely with the MIECHV Coordinator for DCS as well as other state and federal partners to ensure continued successful implementation of MIECHV in Indiana. The MIECHV Coordinator works under the direct supervision of the Home Visiting Team Lead.
Responsibilities :
Development of provider Scope of Work and Service Components for direct services in order to promote and support ISDH MCH implementation of evidence-based and promising practices
Work closely with the NFP National Service Office (NFP NSO) to ensure ISDH funded NFP providers are implementing program with fidelity and to receive Technical Assistance from NFP NSO to support expansion and implementation of the program as needed
Approve provider claims to support appropriate utilization funds
Review effectiveness of provider performance using a variety of methods including regular site visits, report and data reviews and additional reporting mechanisms as available through model owners and other available sources
Review progress reports from providers to monitor achievement toward goals and objectives, provide frequent progress reports to the Home Visiting Team Lead
Coordinate and align ISDH MCH implementation of programs with state and local partners implementing similar programs available in Indiana to ensure all exist as part of a robust continuum of comprehensive systems to maximize resources and impact while avoiding duplication of efforts
Review and maintain familiarity with federal guidance and policies with regards to MIECHV
Development of Benchmark and CQI provider Scope of Work and Service Components in order to promote and support ISDH MCH implementation of evidence-based home visiting practices
Review Benchmark and CQI provider claims to support appropriate utilization of ISDH MCH home visiting funds
Review compliance and effectiveness of Benchmark, CQI and overall MIECHV program performance using a variety of quality assurance and quality improvement methods including active participation in regular data reviews, learning sessions, action periods, studies and development of policy and procedural changes to implement proven improvement strategies
Remain abreast of current literature regarding Early Childhood (EC) systems, NFP and other evidence-based models/promising practices of programs throughout the country. Appropriately relay this information to partners, colleagues and providers.
Work closely with the DCS MIECHV Coordinator as well as ISDH MCH Epidemiologist, Title V Coordinator and Home Visiting Service Coordinator to prepare quarterly and annual reports and future grant applications as required by federal and state guidance.
Preferred Experience:
Seven (7) years work experience in allied health; community health; direct patient health care; health education; hospital administration; public health; or related experience or accredited college training in a related field.
Bachelor?s degree plus minimum of 3 years related experience is required, Master?s degree with related experience is preferred. Areas of study may include public health, health administration, education, healthcare, psychology, social work, or similar disciplines.
Experience implementing Evidence-Based Programs, preferably within an early childhood system and in home &/or community-based settings
Experience coordinating federal grants
Outstanding organizational skills.
Working knowledge of the development of public health strategies for new and existing maternal and child health programs.
Working knowledge of organization and function of ISDH, federal and state government agencies, official health agencies, professional and other groups involved in Maternal and Child Health programs.
Ability to communicate extremely well verbally and in writing - prepare written and oral reports and presentations that communicate necessary information to internal and external partners.
Able to work independently and as a part of a project team.
Able to work under pressure and tight deadlines in a fast-paced environment.
Able to multitask, prioritize, and be flexible.
Proficient in the use of Microsoft Office PowerPoint; Word; Excel or Access.
