OB Care Coordinator Job Opening in Pontiac, Michigan - Company Profile: Oakland Integrated Healthcare Network is a 501c3 Federally Qualified Health Center. We provide integrated, affordable and quality primary medical, dental and behavioral health care to vulnerable populations of Oakland County, regardless of ability to pay.
Oakland Integrated Healthcare Network is seeking an OB Care Coordinator. The OB Care Coordinator is in charge of the day-to-day care and operations of the program. The OB Care Coordinator is responsible for maintaining relationships and growing partnerships with community agencies to generate referrals to the OB program, and link patients with the services and resources these agencies provide. The OB Care Coordinator addresses gaps in care by assisting pregnant women and families in accessing community resources. The Coordinator encourages post-partum and newborn appointments within the health center by making contact post-delivery in hospital. This position requires tracking and follow up of the OB patients ensuring details of care are documented in the Electronic Health Record and population health management tool. Development of reports, data analysis, and follow up are necessary to meet program requirements and improve quality outcomes.
QUALIFICATIONS :
EDUCATION : ADN or BSN registered nurse, bachelors (masters preferred) in human services or similar education required.
EXPERIENCE : A minimum of two (2) or more years in health care environment. Must have basic knowledge of health education elements. Strong background in obstetrics desired. Strong computer skills. Experience with Federally Qualified Health Centers is highly desired.
LICENSURES: Must have a valid Michigan driver?s license with a clean driving record Appropriate State of Michigan Licensure required.
