Discharge Coordinator Job Opening in Country Club Hills, Illinois - Windsor Estates Nursing & Rehab is seeking an experienced, dedicated Discharge Planner to join our team of healthcare professionals for this newly built 200 bed skilled nursing and rehabilitation center.
Requirements and Main Job Responsibilities:
Works Manager on Duty as Assigned
Good communication and Organizational Skills
Social Work experience
Required to know discharge regulations and resident rights
Communicates with insurance companies on resident discharge plans and barriers
Responsible for completion of discharge planning with nursing and therapy department
Completion of MDS Social Service section as needed
Communicates well with community agencies (Hospice and Home Health) and other health care facilities for resident placement.
Active involvement in Care Plan Meetings with families and residents

Required experience:
Qualifications: Job skills or job experiences necessary: successful project planner, extremely organized, proactive, ability to work alone and finish projects, responsible, outgoing & personable to reach out to the necessary resources, good telephone and in person communications and customer service, knowledge of Medicare and Insurance Billing.
Experience:
Basic knowledge of disease processes, ability to recognize problems and to use effective judgment in problem solving.
Knowledge of Microsoft Office (Word, Excel, Outlook)
2 years work experience in a Nursing Home
1 year Social Service Experience
